Pregnancy and Oral Health
In pregnancy there are a lot of physiological changes which are occuring.The emphasis on oral health care during pregnancy and guidance on oral health care should be provided to the patients.All women of child bearing age should be educated about the importance of  oral hygiene during pregnancy.They should have regular dental check up.The treatment such as restorative,preventive can be carried out during pregnancy.Oral health history should be taken from the patient.History of bleeding gums,tooth ache,dental caries,mucosal lesions,signs of infection such as presence of  pus discharge, fistula should be taken.During their treatment on dental chair the position of the patient should be semi recline.Keep patients head at a higher level than feet.If feeling dizzy ,the patient should be turned on left lateral position or  can keep a pillow below the right hip .The pregnant women are encouraged to maintain oral hygiene, eat healthy diet and have regular oral check up. In diet they should eat a variety of healthy food such as fruits,dairy products,cereals,fish ,meat, eggs etc.Food rich in sugar such as candies,cookies, fruit juices,cakes should be avoided.The food should be eaten in small amount and at short intervals.

Local anaesthesia,analgesics,antibiotics can be taken during this period.There are few medication which are not to be taken during pregnancy. Local anaesthesia with epinephrine may be used during pregnancy Antibiotics such as Amoxicillin,Cephalosporin,Clindamycin may be used during pregnancy. MPDS (myofascial pain dysfunction syndrome) is a disorder in which the patient suffers from pain & discomfort in the Temporomandibular joint area , muscles of head, neck & shoulders, and in or around the ear when the patient chews, speaks, or while opening mouth.
Usually the pain is continuous and dull in nature.Sometimes there are trigger -tender points (pain when you press/palpate).There are various factors which cause MPDS.Some of the causative factors include stress,sleep disturbances,injury , mal occlusion ,patients with parafunctional habits like grinding of teeth either day time or while sleeping.
During emotional stress because of hyper functioning of the muscles there is muscle fatigue.Teeth clenching,grinding of teeth ,habit of chewing tobacco or chewing gum can also cause muscle spasm.
Occlusal problems such as faulty restorations,missing teeth for long period can cause occlusal imbalance and changes in vertical dimension.Occlusal disharmony can cause muscle spasm and may lead to pain.
The management of MPDS includes counselling and assurance to the patient.Pharmacotherapy include-analgesic with muscle relaxant,antidepressants. Oral appliance-occlusal splints,night guard can be provided to patients who have a habit of grinding teeth at night. Identification and correction of causative factor should be done . The causative factor are corrected such as replacement of missing teeth with crown ,bridge or denture and orthodontic treatment for correction of malaligned teeth .There are few home measures which patient can practice at home such as use of moist heat or cold packs for local application for about 10 minutes. Patient is advised to eat soft diet.Extreme jaw movements are avoided.Other treatment options include TENS( Transcutaneous electrical nerve stimulation),low level laser therapy LLLT.
